Enrollment Form
For your convenience, we have two options for you.
- The electronic one will allow you fill the form out on line and
email it to us. It contains an electronic signature for you to use and
that eliminates the necessity of you printing the form and manually
signing it. When you click the box, you will be asked some identifying
information; nothing personal, just your name, address etc.. The
program will research that information and gather information, which
someone posing as you would not know. When you have answered the
questions, the program will certify that the person using the digital
signature is in fact, you. When you complete and send this form, your
computer's email program will send it out.
- This is a widely used method of verifying identity and we realize
that some people prefer not to involve themselves with such electronic
“gimmics”. If you fall into that category and we understand
completely.The second option is for you. Please print it, fill it out,
sign it manually and fax it back to us.
